In February 2024, the medical journal BMJ published the largest single accounting yet assembled of what exercise does to depression: a network meta-analysis, meaning a pooled statistical comparison across many trials and many different types of exercise at once, built from 218 randomised controlled trials and 14,170 participants with depression. Michael Noetel and a large international team of collaborators ran the numbers and produced effect sizes for each exercise type, expressed as Hedges' g, a standardized measure of how far apart two groups' averages are, in units of the outcome's own spread. Walking or jogging came out at g = -0.62, with a 95% credible interval of -0.80 to -0.45. Yoga came out at g = -0.55, interval -0.73 to -0.36. Strength training showed a comparable moderate benefit. All three of these numbers describe reductions in depressive symptoms measured against active comparators — not against doing nothing, but against other real treatments or attention-matched controls.
A negative effect size in this context means fewer symptoms, and a magnitude around 0.5 to 0.6 is, by the conventional rules of thumb used across psychology and medicine, a moderate effect — smaller than the difference between a healthy and a badly injured knee, but not a small or trivial one either. Strength training and yoga also had the best acceptability of the modalities studied, meaning the fewest people dropped out partway through, which matters for a treatment that only works if someone keeps doing it. The authors of the 2024 paper flagged their own numbers with an important qualifier, printed in the paper itself rather than added by an outside critic: confidence in the walking/jogging estimate was rated low, and confidence in several of the other comparisons was rated very low. This is not a hedge added for legal cover. It reflects a genuine feature of the underlying evidence — trials in this field tend to be small, and small trials in mental health research have a documented tendency to report larger effects than bigger, more rigorously controlled ones do.
That tendency has its own name in the methods literature — small-study bias — and a separate line of research has gone looking for exactly how much it has inflated the exercise-depression story. In 2016, Kvam and colleagues reassembled the same broad literature and applied a statistical correction called trim-and-fill, designed to estimate and adjust for the effect of unpublished or under-published small negative-result trials. Their uncorrected estimate for the standardized mean difference, a similar measure to Hedges' g, was 0.98; after the correction it rose, not fell, to 1.11, with a 95% confidence interval of 0.79 to 1.43, and a fail-safe N of 1,057 — a separate number describing how many additional null-result studies would need to exist, unpublished, to erase the finding entirely. Taken alone, that would suggest the field has, if anything, underestimated exercise's benefit. But Kvam's broader point, and the point of a separate Cochrane-style systematic review using a technique called trial sequential analysis, was that the heterogeneity across this literature is enormous, and much of it traces back to small studies pulling the pooled average in unpredictable directions. When that second review restricted its analysis to only the trials rated at low risk of bias — meaning trials with adequate randomisation, blinding of outcome assessors, and complete reporting of who dropped out and why — the estimated effect shrank to small and statistically non-significant.
Put the three sets of numbers side by side and the honest summary is not a single clean verdict. Reported effect sizes for exercise against depression range, depending on which trials are included and how publication bias is handled, from roughly -0.4 to as large as -1.4 in different pooled analyses of overlapping literatures. That range itself is the finding. Exercise almost certainly does something to depressive symptoms — no analysis in this body of work, however strict its filtering, finds evidence the effect runs in the wrong direction — but the size of that something is still substantially unsettled, and the field's own most cited numbers should be read with their stated confidence ratings attached, not detached from them.
A different kind of intervention produces an effect that is faster and, in its way, stranger. A single night of total sleep deprivation — staying fully awake through an entire night, with no naps — produces a measurable antidepressant response in roughly 40 to 60 percent of depressed patients, with a pooled average response rate near 50 percent across the trials collected in a 2021 meta-analysis published in Frontiers in Psychiatry. This is not a subtle shift. In responders, mood can lift within hours, a timescale that no standard antidepressant medication comes close to matching, since most pharmacological treatments take two to six weeks to show their full effect. The catch sits in the second half of the same finding: the improvement is fragile. After a single night of recovery sleep, up to 83 percent of the people who had responded relapse back to their prior depressive state. The effect can be stretched out — combining sleep deprivation with standard antidepressant medication, or with other circadian-timed interventions such as shifting the sleep schedule earlier or adding bright light exposure, reduces the relapse rate and can prolong the benefit — but on its own, one night without sleep buys, for many people, only a day or two of relief before the prior state reasserts itself. The mechanism behind the rapid effect is still an open question rather than a settled one; researchers have proposed explanations involving a resetting of circadian gene expression and involvement of inflammatory or immune signaling pathways, but neither account is yet confirmed. What is well established is the shape of the effect itself: fast onset, roughly coin-flip odds of response, and a relapse curve steep enough that the finding is treated in the literature as a research tool illuminating how mood systems can shift quickly, rather than as a viable treatment on its own.

Light exposure is the third variable this chapter can report on with numbers attached, and the finding that has drawn recent attention concerns non-seasonal depression specifically — major depressive disorder occurring outside the autumn-winter pattern that bright light therapy was originally developed to treat. A systematic review and meta-analysis published in late 2024 pooled 11 randomised clinical trials with more than 850 participants total, comparing a standard protocol of at least 30 minutes per day of exposure to a 10,000-lux fluorescent light box against several types of control condition, including antidepressant medication alone, dim red light, and a negative-ion-generator device used as an inert-seeming placebo. Bright light therapy, whether used alone or added on top of an antidepressant, was associated with a 41 percent remission rate in non-seasonal depression, meaningfully higher than the comparator treatments. A separate meta-analysis of 15 randomised trials found the same general direction of effect on depressive symptoms and response rates. An earlier randomised trial that compared light therapy alone, the antidepressant fluoxetine alone, and the two combined found that the combination outperformed either treatment given by itself — light functioning, in that trial, as an addition to pharmacological treatment rather than a replacement for it. Across this literature, no serious dispute was found over the basic direction of the effect for non-seasonal depression; the open questions concern dose, duration, and timing of exposure, which remain under active study rather than standardized.

None of these three levers — movement, sleep loss, light — operates through a mechanism fully mapped end to end, and none of the numbers above should be read as larger or more certain than the studies that produced them. What they share is a common shape: each is a measurable input from outside the skull that shifts a measurable signal inside it, on a timescale and with a caveat that differs for each one, and each has been checked, at least once, against the specific possibility that its apparent effect was an artifact of how the studies measuring it were built.
Overturned if: a large, pre-registered, low-risk-of-bias trial of aerobic exercise against an active control finds an effect on depressive symptoms statistically indistinguishable from zero, converging with the low-risk-of-bias-restricted finding already reported and resolving the current range of pooled estimates toward the null rather than toward the moderate effect sizes reported in the 2024 network meta-analysis.
