03. The Cost of Chronic Vigilance
A smoke alarm exists to fire once, briefly, and stop. It is calibrated for an emergency that resolves — the fire is put out, the toast is scraped off the pan, the room clears, and the circuit resets. A smoke alarm that has lost its off switch, chirping steadily through an ordinary afternoon with no fire anywhere, is not protecting anyone. It is simply running down its own battery while everyone in the house adapts to the noise. The body's stress-response system was built on the same one-shot logic. In depression, in prolonged trauma, in the kind of vigilance that never gets the signal to stand down, that system can behave like the alarm with the stuck switch — and the wear from that is not metaphorical. It shows up, measurably, on a brain scan.
The system in question is called the hypothalamic-pituitary-adrenal axis, or HPA axis, named for the three structures that pass a signal down a chain: the hypothalamus at the base of the brain, the pituitary gland just beneath it, and the adrenal glands that sit atop the kidneys. Under threat, the hypothalamus signals the pituitary, the pituitary signals the adrenals, and the adrenals release cortisol, a hormone that mobilizes energy, sharpens attention, and suppresses functions the body can temporarily do without, such as digestion and long-term tissue repair. In a healthy system this cascade also carries its own brake: rising cortisol feeds back on the hypothalamus and pituitary and tells them to stand down, so the alarm shuts itself off once the threat has passed. Carmine Pariante and Stafford Lightman, at King's College London and the University of Bristol, reviewed four decades of research on what happens to this circuit in major depression and reported one of the field's most consistent findings: the HPA axis in depressed patients runs hot. Cortisol is elevated in saliva, blood, and urine. The pituitary and adrenal glands themselves are measurably enlarged, as though built up from overuse. And the feedback brake — the mechanism that is supposed to switch the alarm off — does not engage properly.

What Pariante and Lightman's review makes clear, and what deserves to be said plainly rather than glossed over, is that the reproducibility of the finding and the explanation for it are two separate things. That cortisol runs high in depression is not seriously contested; it has shown up across forty years of studies using different methods on different populations. Why the feedback brake fails is the genuinely open question their review is built around. One candidate explanation they weigh is glucocorticoid receptor resistance — the idea that the cells meant to sense circulating cortisol and pass the "stand down" signal upstream become less responsive to it over time, so the brake pedal is being pressed but the car no longer slows. Other candidate pathways compete with that one. At the time of their 2008 review, the field had not settled which mechanism, or combination of mechanisms, was doing the work, and Pariante and Lightman organize their paper explicitly as a comparison of older, classical explanations against these newer candidates rather than a declaration that the matter is closed.
One of the tissues sitting downstream of chronically high cortisol is the hippocampus, a seahorse-shaped structure buried in each temporal lobe that is central to forming new memories and, notably, carries an unusually dense concentration of receptors for cortisol itself — which makes it one of the brain regions most directly exposed to whatever cortisol is doing, in the same way a shoreline is exposed to a tide that inland ground never feels. Robert Sapolsky, at Stanford, spent much of his career tracing what sustained glucocorticoid exposure — glucocorticoid being the class of hormone cortisol belongs to — does to that structure, building on an earlier glucocorticoid cascade hypothesis he had proposed years before with Krey and McEwen, describing a feedback loop in which stress hormones damage the very brain tissue responsible for helping shut the stress response off, potentially making each subsequent stress response harder to end. In a 2000 review, published in Archives of General Psychiatry, Sapolsky pulled together evidence from three quite different conditions that share one thing in common: abnormally high glucocorticoid exposure. Cushing syndrome is a disease in which the body overproduces cortisol directly, unrelated to mood at all — a useful natural experiment, since whatever hippocampal change shows up there cannot be explained by depression itself. Recurrent major depression, per Pariante and Lightman's findings, runs an overactive HPA axis of its own. Post-traumatic stress disorder involves its own separate pattern of stress-hormone dysregulation, tied to a different triggering history entirely. Three conditions, three different origins, one shared exposure. In all three, independently, hippocampal volume comes out smaller on brain scans than in matched people without the condition.

A convergent pattern across three unrelated illnesses, all sharing the same hormonal exposure, is suggestive of a causal mechanism — glucocorticoid toxicity, damage accumulating in a vulnerable tissue under chronic hormonal load — but Sapolsky's own review is explicit that convergence across conditions is not proof of mechanism within any one of them. That caveat turns out to matter a great deal for how the next, more specific finding should be read.
Poul Videbech and Barbara Ravnkilde, at Aarhus University Hospital in Denmark, narrowed the question to unipolar depression specifically and ran a formal meta-analysis, pooling twelve MRI volumetric studies covering 351 depressed patients against 279 healthy controls, published in 2004. The pooled result was a real, statistically robust deficit: hippocampal volume averaged 8 percent smaller on the left side and 10 percent smaller on the right, with standardized effect sizes of −0.38 on the left and −0.32 on the right, both confidence intervals excluding zero. A further analysis within the same data found that the size of the deficit tracked the number of depressive episodes a patient had experienced — a statistically significant correlation on the right side (r = −0.30) — rather than tracking age or whether it was a first episode. People with more depressive episodes behind them had smaller hippocampi. People with bipolar disorder, examined across the same pool of studies, did not show the same pattern, which argues against a generic "any mood disorder" explanation and for something more specific to the recurring stress load of unipolar depression.
What the Number Does Not Prove
That 8-to-10-percent figure needs one immediate qualification before it travels any further, because a later, more targeted meta-analysis found something that changes what it means. Restricting the analysis to first-episode depression — patients scanned at or near their first depressive episode, rather than the mixed recurrent samples in the Videbech and Ravnkilde pool — a 2012 meta-analysis associated with King's College London found a much smaller deficit: roughly 4.0 percent on the left and 4.5 percent on the right, about half the magnitude reported for recurrent depression. Read next to each other, the two findings are not in conflict; they are compatible with an illness that accumulates its physical cost with each recurrence rather than one that starts with a fixed, pre-existing volume loss. The headline 8-to-10-percent number is not a fixed property of "having depression." It is closer to a running total.
The largest caveat in this chapter is not about magnitude, though, but about direction, and both of the primary sources say so themselves, in their own words, rather than leaving it to later commentary. Videbech and Ravnkilde pose the open question directly: does the depression cause the hippocampus to shrink, or does having a smaller hippocampus to begin with make a person more vulnerable to becoming depressed? A cross-sectional MRI study — one that measures a group of people at a single point in time — cannot distinguish between those two stories. A smaller hippocampus measured in a depressed adult could be the scar of the illness, or it could be a preexisting vulnerability that the illness later exploited, or some mixture of both operating on different people. Sapolsky's earlier review frames the same limit around Cushing syndrome and PTSD: association across conditions sharing a hormonal exposure is not the same evidence as watching one structure change over time inside one person. Both sets of authors call, explicitly, for longitudinal studies — the kind that scan the same people repeatedly, before and after illness onset, rather than comparing two different groups once — as the only design capable of settling which direction the arrow runs.
That is the honest state of the evidence, and it resists the tidy version of the story. The stress-alarm system in depression does appear to run without its brake, measurably, on cortisol assays taken from thousands of patients over four decades. Brain tissue in a region saturated with cortisol receptors does come out smaller on the scans of people who have lived through more depressive episodes. What the data cannot yet show is whether the alarm wears down the house, or whether some houses were built with a lower tolerance for the alarm going off in the first place. Both possibilities remain open, side by side, in the same set of numbers.
Overturned if: a longitudinal MRI study — scanning the same individuals before and after a first depressive episode — finds hippocampal volume unchanged by the episode, indicating the smaller volume seen cross-sectionally was present beforehand rather than caused by the illness.
