05. Rapid Restoration
A suspension bridge with a handful of snapped cables does not collapse right away. It sags. Traffic slows. The whole structure still stands, technically functional, carrying less weight than it was built for, waiting either for repair or for the next cable to go. Standard antidepressants, on the available evidence, work like a slow repair crew: they arrive, and over four to six weeks they gradually re-tension what has slackened. Some patients cannot wait four to six weeks. Some do not have that runway left.
In 2000, a small team at Yale — John Krystal, Dennis Charney, Robert Berman and colleagues — tested whether a drug with no prior history as an antidepressant could shorten that timeline drastically. The drug was ketamine, long used in operating rooms as an anesthetic and, at much lower doses, studied here for its psychiatric effects. Nine depressed patients received a single low intravenous dose, 0.5 milligrams per kilogram of body weight, in a double-blind trial against placebo. On the 25-item Hamilton Depression Rating Scale, a standard measure of depressive symptoms, scores in the ketamine group fell by an average of 14 points within 72 hours; the placebo group showed no meaningful change. Half the ketamine patients met the threshold for a treatment response. Only one of eight on placebo did. It was a tiny study, nine people, explicitly framed by its own authors as a proof of concept rather than a confirmatory result — but it was the first placebo-controlled, double-blind demonstration that a drug acting on a completely different chemical system, glutamate, could lift depression, and lift it in days rather than weeks.
The finding needed replication at larger scale and in a harder population, and it got both in 2006, when Carlos Zarate and colleagues at the National Institute of Mental Health ran a randomized, placebo-controlled crossover trial in eighteen patients with treatment-resistant major depression — people for whom conventional antidepressants had already failed. Each patient received both ketamine and placebo, on separate occasions, and was rated at multiple points from 40 minutes out to a full week. The effect was not subtle. At 24 hours, the statistical effect size was 1.46 — large by the standards of psychiatric trials, where an effect size around 0.3 is often considered clinically meaningful. Seventy-one percent of ketamine-treated patients met response criteria the day after infusion; 29 percent met full remission. This is the benchmark trial for the field, the one that established "hours, not weeks" as ketamine's defining clinical fact.
It also established the field's defining problem, in the same data set. By one week, the effect size had fallen to 0.68. Only 35 percent of patients who had responded were still responding seven days later. Most of the people who got dramatically better within a day or two had relapsed by the end of the week. The 2006 trial is remembered for showing that ketamine works fast. It is equally a record of ketamine wearing off fast — a fact its own authors reported openly, not something uncovered later by critics.
A further concern with the early trials was that ketamine's placebo comparison was, in a sense, too easy. A saline injection produces no sensation at all, while ketamine produces a distinctive, hard-to-miss dissociative state — a feeling of detachment from one's body or surroundings. A patient does not need a doctor to tell them which arm of the trial they were in; a sizeable subjective experience tells them itself. In 2013, James Murrough and colleagues, running a two-site trial at Mount Sinai and Yale, tried to control for this by comparing ketamine against midazolam, a sedative that produces its own noticeable effects without the same antidepressant properties — an "active placebo" meant to at least partially disguise which drug a patient had received. Seventy-three patients were randomized, two to one, toward ketamine. On the primary measure, the Montgomery-Åsberg Depression Rating Scale, the ketamine group scored 7.95 points lower than the midazolam group at 24 hours, and 64 percent of ketamine patients showed a response by that point. The effect held up even against a comparator designed to mimic some of ketamine's own side effects — reassuring, as far as it went.
It did not, as it turned out, go far enough. A drug's mechanism was, by 2019, understood in considerable structural detail. Nanxin Li and Ronald Duman, working with rats, traced a specific molecular chain: ketamine triggers a surge of the neurotransmitter glutamate, which activates a receptor called AMPA, which in turn switches on a signaling pathway called mTOR — and mTOR activation is what drives the rapid construction of new dendritic spines, the same physical connections that chapter four described as thinned out by chronic stress. When the researchers chemically blocked mTOR, both the new spine growth and the antidepressant-like behavioral effect in the rats disappeared completely. This is a causal chain, not a correlation: block one link, and the whole downstream effect vanishes. It gave ketamine's rapid mood effect a physical mechanism to match the animal-tissue story of chapter four — the drug appeared to be regrowing, within hours, connections that chronic stress had taken weeks or months to strip away.

A 2019 study led by Conor Liston at Weill Cornell, working with Duman as a senior collaborator, sharpened the picture further and, in doing so, explained the relapse problem directly. In mice exposed to three weeks of chronic stress hormone, ketamine's behavioral antidepressant effect showed up within three hours — fast, as expected. But new spine formation in the prefrontal cortex was not measurably elevated until 12 to 24 hours after treatment, and it was this later spine growth, not the immediate chemical effect, that determined whether the improvement lasted. Mice in which the new spines were experimentally erased relapsed; mice that kept them stayed well. The initial lift and the durable recovery turned out to be two separate biological events running on two different clocks, and only one of them builds anything that survives past the first day.

This mechanistic story helped ketamine cross a regulatory threshold. On March 5, 2019, the FDA approved esketamine, sold as a nasal spray under the name Spravato, for treatment-resistant depression — the first NMDA-receptor-targeting antidepressant to reach market, taken alongside an existing oral antidepressant. The approval was real, but not uncomplicated: it moved through an accelerated breakthrough-therapy pathway, and among the phase 3 trials submitted, some failed to separate from placebo — a detail regulatory critics have pointed to since.

The blinding problem that will not go away
The active-placebo trials of the 2010s were meant to settle the question of whether ketamine's benefit was pharmacological or simply the result of patients realizing, from the drug's unmistakable effects, which arm of the study they were in. A newer, more direct line of evidence suggests they did not settle it. A systematic review of blinding integrity across psychedelic and dissociative drug trials found that only 17.9 percent of ketamine trials had formally assessed whether their blinding actually held. In one recent midazolam-controlled serial-infusion trial that did check, participants who received ketamine correctly guessed their assignment 78.1 to 85.0 percent of the time across measurement points; participants on midazolam guessed correctly less often, 46.4 to 61.5 percent, but still well above chance. A separate 2021 paper found that the intensity of dissociation itself was linked to the size of the antidepressant response — raising the possibility that some portion of what looks like a drug effect is an effect of noticing, unmistakably, that something powerful has happened. This is not a fringe objection. It is acknowledged inside the field as an open design problem, one motivating a newer generation of trials built specifically to be blinding-resistant. Dissociation of this kind is also part of why ketamine carries a recognized potential for recreational misuse, separate from its clinical use under supervision.
What remains, after all the caveats: a drug that reliably produces measurable antidepressant effects within hours in controlled trials, through a documented molecular pathway that regrows the very connections chronic stress destroys — and a benefit that tends to fade within days unless something sustains the new growth, measured by trial designs whose blinding, on the best current evidence, is far from airtight.
Overturned if: a trial using a validated blinding-integrity check found ketamine's antidepressant effect size collapsed to the level of an inert placebo once analysis was restricted to participants who could not correctly guess their treatment assignment — showing the effect depends on knowing, not on the drug.
